To claim on your hospital benefit you can send us the original discharge letter provided to you by the hospital. Alternatively, please complete the 'patients details' section on the back of the paper claim form and ask a hospital representative to complete the 'Admission details'.

All future claims will then be paid directly into your nominated account.
Claims history for your account can be viewed online once you have registered with Online Self Service.

Order a claim form today and we'll send it to you in the post within five working days. All you have to do is register or log in to Online Self Service to request a personalised form or gain access to our new printable version of the form. Simply complete your personalised claim form online, print and pop it in the post along with your receipt.
