   You'll receive money back in your bank account usually within a few days 

To claim on your hospital benefit you can send us the original discharge letter provided to you by the hospital. Alternatively, please complete the 'patients details' section on the back of the paper claim form and ask a hospital representative to complete the 'Admission details'.

Log in or register for Online Self Service today and complete the online form and we'll set up your direct credit within 7 working days. Alternatively, you can call our Customer Services team.

All future claims will then be paid directly into your nominated account.

Claims history for your account can be viewed online once you have registered with Online Self Service.

Request a claim form

Order a claim form today and we'll send it to you in the post within five working days. All you have to do is register or log in to Online Self Service to request a personalised claim form or complete the online claim form.  Alternatively, give our customer services team a call on  0370 908 3481.
